Several changes have been announced to insurance-based fitness memberships for 2025. Effective January 1, Medicare-eligible Kaiser Permanente members will transition away from SilverSneakers and move to Renew Active One Pass. In addition, some United Healthcare HMO plans will move to SilverSneakers.

If your insurance is changing or you are newly eligible for a fitness membership through your healthcare insurance provider, please fill out the Insurance Based Fitness Membership Form so we can begin the registration process.
